Vancomycin is one of the most commonly used antibiotics in hospitals today. Dosing and monitoring of vancomycin can be complex, requiring clinicians to take many considerations into account including patient demographic data and laboratory results to individualize dosing for each patient. More importantly, in 2020, vancomycin therapeutic guidelines recommended moving away from monitoring vancomycin trough levels to targeting area under the curve (AUC), with preference given to utilizing a Bayesian software program to monitor the AUC.
