The first appearance of Bowties in an Italian book about Risk Management

We are very excited to announce that the renowned bowtie method is present in a new Italian book about Risk Management. We want to congratulate our dear partners Rosario Siscari from Chaos Consulting en Luca Fiorentini from TECSA on the release of their new book “Analisi, valutazione e gestione operativa del rischio”. The book is written in Italian and deals with bowties, root cause analysis, and other ISO 31000 compliant tools for systemic risk management in organizations.

Abstract

Any organization, regardless of its size and the sector to which it belongs, is called upon to manage its own risks, whether they are connected with economic, environmental, or health and safety aspects, guaranteeing their keeping over time within acceptable levels. Starting from the requirements of ISO 31000 in terms of risk management, after analyzing the criteria deriving from the main standards of our country, this book illustrates methods and tools for conducting both risk assessment and treatment phases. After illustrating the main risk identification, analysis, and quantification techniques, ample space is devoted to the “barrier-based” perspective and the “Bow-Tie” methodology, illustrating the criteria for the autonomous construction of a Bow-Tie diagram and the steps to be followed to manage risks with this tool, both for qualitative and quantitative assessments thanks to the integration with the LOPA analysis. A specific chapter is dedicated to the analysis of the operational experience of the organizations, where the related connections with the various mandatory or voluntary management systems are highlighted, giving importance to the complementarity with the previous risk assessment phase. The investigation workflow is then illustrated, along with the fundamental principles of forensic engineering. Ample space is dedicated to various accident analysis techniques, near miss or non-compliance, such as time-line, Tripod Beta, BFA, RCA, and others. Finally, a special chapter deals with the theoretical principles of the human factor, with insights dedicated to the techniques for quantifying human reliability.
