As part of a recent call for ideas/Hackathon initiative held by Piedmont Regional Healthcare Authority, we explored how the integration of clinical decision support and digital and logistic solutions could help improve prescription and distribution processes for those living in nursing homes in Italy. Our winning proposal is detailed below.
The challenge
General practitioners (GPs) are responsible for prescribing medications for nursing home residents, but it’s been observed that some GPs don’t adhere to the indication regulated by the Prontuario farmaceutico aziendale (pharmaceutical formulary) as well as to the correct process for drugs distribution to these patients.
Our observations
- Limited awareness: Many GPs are unfamiliar with nursing home-specific prescription guidelines.
- Lack of support tools: There is no system to guide GPs towards appropriate clinical and formulary-compliant prescriptions.
- Inefficiencies in distribution: Errors, delays, and non-availability of medications can disrupt the prescription and delivery process.
- Costly duplications: When prescriptions don’t adhere to direct distribution protocols, it results in higher pharmaceutical spending.
In the Piedmont Region in Northern Italy (~4.25m inhabitants) these issues are not related to specific areas; across the territory of the Città di Torino Local Health Authority (LHA) alone, 70 nursing homes highlighted similar challenges, signalling a broader systemic problem.
This is where the Hackathon on Innovative Logistic’s proposal comes into play. By integrating clinical decision support and logistic systems online, this solution aims to create a more effective framework for managing medications in nursing home environments.
