Case study: UAB admissions

UAB challenged us to extend their reach post discharge, reduce staff time spent connecting with low risk patients, and help discharged patients self-manage chronic conditions. This case study discusses how we measured engagement rates, helped care teams become more efficient, and ultimately helped UAB achieve lower readmission rates.
