Retail pharmacy’s next chapter: Insights from NACDS 2025

Retail pharmacies are rapidly evolving into community care hubs, and new survey data from the 2025 NACDS Total Store Expo reveals what’s driving this transformation.

Leaders across the industry shared their biggest challenges and most promising opportunities, and the results point to a future shaped by clinical expansion, smarter technology, and streamlined operations.

Expanding clinical services

Pharmacies are expected to provide more than simply filling and dispensing prescriptions. Services like immunizations, chronic disease monitoring, and point-of-care testing are becoming standard. But the shift isn’t easy.

Workforce shortages (41%) top the list of barriers, followed by budget constraints (35%) and lack of technology tools (6%), according to survey respondents. With limited staff and tight margins, expanding clinical roles requires strategic investment in both people and infrastructure.

Leveraging technology for pharmacy transformation

Technology is key to unlocking efficiency and enabling clinical care. Respondents said workflow automation would have the greatest impact on operations as automating routine tasks allows pharmacists to focus on higher-value services.

Better integration with local care networks (35%) was also a priority, helping pharmacies become more connected to the broader healthcare system. Despite the clear benefits, adoption is slow—cost (35%), resistance to change (29%), and uncertainty about ROI (29%) remain major hurdles.

To move forward, leaders must choose solutions that are easy to implement, deliver measurable value, and come with strong vendor support.

The vision: Community care hubs

Pharmacy leaders aren’t thinking small. When asked which service is most valuable for positioning their pharmacy as a care hub, 88% chose “All of the above.” That includes preventive care, wellness counseling, immunizations, and chronic disease management.

This ambitious vision reflects a growing consensus: pharmacies are uniquely positioned to deliver accessible, high-impact care. However, achieving this requires pharmacies to leverage technology and manage operations.
